CAN MENSTRUAL BLEEDING BE AVOIDED?
My catch phrase for the 1990s is "designer HRT". This refers to the fact that today every woman who wants HRT can have it tailor made or designed to suit her unique needs. A lot of older post-menopausal women stop HRT because they don't want monthly menstrual bleeding. This is a pity as designer HRT can reduce or completely eliminate menstrual bleeding.
Nowadays older women are often prescribed both an oestrogen and progesterone tablet every day without a break which completely avoids menstrual bleeding. We'll soon have a single HRT pill combining the two sex hormones oestrogen and progesterone which can be taken every day without a break avoiding any menstrual bleeding. If any spotting or breakthrough bleeding occurs on this program you may take a seven-day break from the oestrogen and progesterone tablets every three months which would enable your uterus to shed any built-up lining in the form of a light menstrual period. Some women feel more comfortable having a menstrual bleed every three or so months, seeing it as a ' 'spring clean'' for the uterus after which they often feel better.
In women over the age of 65, if it is thought necessary to begin HRT for the first time to slow the progression of osteoporosis, a small dose of oestrogen may be given continuously with no breaks at all. In such cases, a light menstrual bleed can be brought on only two or three times a year by taking a two-week course of progesterone tablets. This reduces the annoyance of regular monthly bleeds in older women.
From table 4 you can see that daily dosages of oestrogen tablets vary considerably in amount, depending upon how rapidly your enzymes break them down and also upon which brand of oestrogen you are prescribed. For example, Ogen is three times stronger than Ovestin, Progynova is ten times stronger than Ovestin and Estigyn is three hundred times stronger than Ovestin. In all cases you must be guided by your own doctor in finding your required dose.
